HealthScreen Disease Management Announces Graduated Levels of Care

HealthScreen Disease Management, an independent health risk management company specializing in Population Health Analysis and Care Management for mid-large-sized self-insured employer groups, announced today that participants in their innovative chronic disease management programs now have the opportunity to graduate as they progress and demonstrate improvement in their condition state. With HealthScreen, each program participant has their own personal nurse care manager who coaches and educates the patient about their condition, gauges medical compliance and assists with behavior modification toward a healthier lifestyle. The patient'ss physician always remains in charge of the treatment plan. Waived and/or reduced prescription drug co-payments are used as an incentive or, in some groups, reduced health plan premiums. Participants also receive free condition monitoring devices, as appropriate (glucometers for diabetics, blood pressure cuffs for hypertensives, etc.).

When the nurse care manager has evaluated a participant as being stable: remaining compliant, meeting certain goals, clinical indicators and behavioral disposition, the participant will move into the next appropriate Level of Care (three levels in all) which reduces the call/encounter frequency. This usually occurs anywhere from 6-12 months after a first round of constant, consistent coaching. We never leave a patient in a lurch, so to speak, which can result in a relapse. We continue to monitor claims and of course the participant can always make in-bound calls to their care manager, said Clinical Care Coordinator, Jan Kiss, R.N.
